guessing with subjective grading scales. Using
them pre-operatively, different injury patterns
and instability grades can be characterized
based on patient factors. This can help us to
devise individualized treatment plan for the
patient. The future application of widespread
quantitative evaluation technologies will help
correlate patient reported outcome with
objective findings of knee instability with the
goal of improved patient outcomes.
SUMMARY
Pivot shift test is the most specific tool for
assessment of anterolateral instability of the
knee. It has application at all levels of care in
ACL injured patient, right from pre operative
phase to return to play decisions. Various
anatomical and morphological factors can
enhance or mask the pivot shift. Use of
Standardized Pivot shift technique can bring
uniformity to testing techniques. Quantitative
assessment of pivot shift helps with the
development of individualized treatment algo
rithm for the ACL-injured patient with the goal
of achieving maximum athletic potential while
at the same time preventing post-traumatic OA.
In the future, quantitative pivot shift testing
will aid surgeons in their indications for
additional procedures such as extra articular
tenodesis or meniscus allograft reconstruction.
